The mid mount aerial ladder or tower is still quite popular here in the US, even though this design results in a very long vehicle and almost always requires tandem rear axles - as, of course do many rearmounts.

The big advantage is height - the truck is a lot lower than a rearmount, and this really counts when it has to be shoehorned into an old firehouse's appliance room with low doors!

Even though tandem axles with a trailing steering axle are quite common on aerials in Europe, this technology does not seem to have penetrated the fire truck industry here.
